แบบฟอร์มสมัครสมาชิกห้องสมุด

*คำชี้แจง: สิทธิ์การสมัครสมาชิกห้องสมุดเป็นไปตาม ประกาศ หอสมุดและคลังความรู้มหาวิทยาลัยมหิดล เรื่องการกำหนดอัตราค่าบริการของห้องสมุด พ.ศ. ๒๕๖๕ ห้องสมุดจะเก็บข้อมูลของท่านเพียงเท่าที่จำเป็นในการปฏิบัติงาน และจะรักษาข้อมูลของท่านเป็นความลับ การเปิดเผยข้อมูลใดๆ ของท่านต้องเป็นไปตามกฎหมายที่เกี่ยวข้อง

ทั้งนี้ หอสมุดและคลังความรู้ฯ จะดำเนินการจัดเก็บข้อมูลส่วนบุคคลของท่านเท่าที่จำเป็นต่อการดำเนินงาน ภายใต้นโยบายคุ้มครองความเป็นส่วนตัว หอสมุดและคลังความรู้ฯ รวมถึงตามกฎหมายที่เกี่ยวข้อง และประมวลผลข้อมูลส่วนบุคคลของท่าน เพื่อดำเนินการตามวัตถุประสงค์ของหอสมุดและคลังความรู้ฯ ขอให้ท่านโปรดศึกษารายละเอียดได้ที่ พรบ ข้อมูลส่วนบุคคล , ประกาศ นโยบายฯ มหาวิทยาลัย , ประกาศนโยบายฯ หอสมุด
