*คำชี้แจง: แบบฟอร์มการแจ้งปัญหานี้มีการเก็บข้อมูลส่วนบุคคล ได้แก่ ชื่อ-สกุล อีเมล รหัสบัญชีเข้าใช้งานอินเทอร์เน็ตของมหาวิทยาลัยมหิดล สถานะ สังกัดส่วนงาน เพื่อแจ้งปัญหาที่พบจากการเข้าใช้ฐานข้อมูลฯ , ตรวจสอบสิทธิ์การเข้าใช้งาน และการติดต่อกลับจากเจ้าหน้าที่
หอสมุดและคลังความรู้ฯ จะดำเนินการจัดเก็บข้อมูลส่วนบุคคลของท่านเท่าที่จำเป็นต่อการดำเนินงาน ภายใต้นโยบายความเป็นส่วนตัว (Privacy Notice) หอสมุดและคลังความรู้ฯ รวมถึงตามกฎหมายที่เกี่ยวข้อง และประมวลผลข้อมูลส่วนบุคคลของท่าน เพื่อดำเนินการตามวัตถุประสงค์ของหอสมุดและคลังความรู้ฯ ขอให้ท่านโปรดศึกษารายละเอียดได้ที่ พรบ ข้อมูลส่วนบุคคล , ประกาศ นโยบายฯ มหาวิทยาลัย , ประกาศนโยบายฯ หอสมุด
